However, it’s important to note that erection occurs only in case nitric oxide is released from endothelial cells in the penile tissue and neurons, which is possible only during sexual stimulation. Nitric oxide activates guanylyl cyclase, the enzyme that helps convert guanosine triphosphate into cGMP. This process of conversion results in activation of protein kinase, diminishing of calcium levels in the cells, phosphorylation of a few proteins, smooth and slow smooth muscle relaxation and increased flow of the blood resulting in corpus cavernosum enlargement. As a result of increased tumescence the veins between tunica albuginea and corpus cavernosum compress, and this provokes a stiff erection. Therefore, vardenafil is only effective if there is sexual arousal.
